These vouchers apply if you were injured in 2004 or later and you have a permanent partial disability.

How Do I Obtain the Benefit and Receive the Voucher?
A claims administrator is required to offer this benefit to you if your injury causes permanent partial disability and if your employer does not offer you regular modified, or alternative work within 60 days after the claims administrator receives a Return-to-Work & Voucher Report. You must be offered the voucher within 20 days after the end of the period when your employer may offer you regular, modified, or alternative work. You must always be alert to the fact that there is a deadline on your voucher, however. It expires two years after the date it is furnished to you, or five years after your date of injury. It really depends on whichever is later.

What Rates Are SJDBs Based On?
Basically, your permanent disability level will determine your SJDB voucher amount. If it is less than 15%, you can receive up to $4,000. If it is 15% to 25%, you can receive up to $6,000. If it is 26% to 49%, you can receive up to $8,000. And if it is 50% to 99%, you can receive up to $10,000.

How Do I Use the Voucher?
First of all, you can only use the voucher to help you pay for and attend a California public school or receive training with a provider on the state’s special list known as the EPTL. But how do you use it? You must present the voucher to the school and counselor of your choice so that they can contact the claims administrator for direct payment. If you submit receipts to the claims administrator, they must reimburse you within 45 days after receiving the receipts along with your signed voucher.
